PDFCreator 3.0
When using the Windows© Explorer to open a document from the context menu, it returned “Failed to open Documentâ€; however if we open the document from the Creator it open fine.
Any suggestions?

Hello,
You mean if you double click on a PDF document to open it you get "Failed to Open document" and if you open the PDF Creator and use File -> Open to open the PDF it opens properly?
Please check the following:
1 - Which application is associated with the PDF file to open it?
2 - If the PDF Creator is associated with the PDF documents, please make sure that you don't have 2 versions of the PDF Creator on your system
Which version 3 are you using? we are currently at 3.02
