Creating/Printing Claim Forms from a Database

I need to print medical claim forms (UB-04 and HCFA1500) from a database to PDF. My goal is to make a print routine that will print all the data in the correct form locators from any printer. Can anyone comment on the best way to go about this?
I see that I can use PDF creator to prgramatically layout a form and assign values to the text fields. It looks like another option is to create a fillable form and then use an FDF file to merge the data. Is one of these approaches better than the other? Are there others? Are these within the scope of what can be done with PDF Creator and Converter?
